ClinicalForm.com and Concussion Assessment

While there has been a recent flurry of news and interest in concussion assessment, few robust tools exist to obtain and track assessments for these patients. Our Clinical Forms software will now include a few common assessments used both on the sideline and for follow-up monitoring of concussion patients. CES 2012 – Clinical Hardware

With the first day of CES starting, we are excited to be in Las Vegas and reviewing some of the newest clinical hardware in addition to having some of our products on hand. Minnesota Community Measures

Minnesota is one of the first states to mandate reporting for the care of patients that fall into several categories. Providers in Minnesota who see patients with depression, asthma, vascular disease, diabetes, and those who require screening for colorectal cancer must report on a set of standard measures to the Minnesota Community Measures (MNCM) program. While this program may provide several benefits from a population care perspective, tracking and managing this data is time intensive and difficult for many providers. Additionaly, many fields that must be reported to the MNCM system are not commonly collected in existing electronic medical record (EMR) systems and are not coded correctly for reporting. Clinical Forms by AgileMedicine allows providers to track and capture data as required by the MNCM program for patients with depression (PHQ-9 screening) or asthma. Our intuitive, online system allows providers to fit this data collection into their existing workflow and quickly export the data in the format required by MNCM, allowing providers to focus on providing the best clinical care to individual patients, while allowing our system to track population health. This cheap method for digitally managing medical forms and population-based health information is the first software to monitor patients for the MNCM requirements and mandate.
