Minnesota is one of the first states to mandate reporting for the care of patients that fall into several categories. Providers in Minnesota who see patients with depression, asthma, vascular disease, diabetes, and those who require screening for colorectal cancer must report on a set of standard measures to the Minnesota Community Measures (MNCM) program. While this program may provide several benefits from a population care perspective, tracking and managing this data is time intensive and difficult for many providers. Additionaly, many fields that must be reported to the MNCM system are not commonly collected in existing electronic medical record (EMR) systems and are not coded correctly for reporting. Clinical Forms by AgileMedicine allows providers to track and capture data as required by the MNCM program for patients with depression (PHQ-9 screening) or asthma.
