Expansion of Clinical Forms – Alcohol Use History

A large number of colleges and universities are beginning to use alcohol screening assessments during routine evaluations at their health clinics. In response to this interest, we have added the Alcohol Use Disorders Identification Test (AUDIT) to Clinical Forms. The AUDIT allows providers to quickly assess alcohol dependence in ten routine questions. As recommended by the World Health Organization, this form can be completed electronically by the patient. Clinical Forms by AgileMedicine allows clinics and health care providers to send out electronic assessments by e-mail prior to appointments, meaning the form can be filled out without taking time away from the office visit!
